To help you properly, our practice assistant asks about the reason for your contact. She does so to work out together with you how we can best help you and at what time. The practice assistant is trained in this and treats your information confidentially.
The practice assistant is the first person to speak to you when you call the practice. She makes appointments for consultations and home visits. The assistant can provide independent medical advice for common problems such as fever, sore throat or diarrhoea. You can also visit by appointment for:

The practice is only open to new patients who do not yet have a GP in Amstelveen and live in postcode area 1183. To register, you can fill in a registration form at the front desk in the morning between 8.30 and 11.30 am. You should bring proof of identity and BSN number and, if possible, your health insurance card.
The results of (laboratory) tests can be viewed in the patient portal.
In addition, you can request the results from your GP during the telephone consultation between 13:00 - 14:00. It is also possible to discuss the results during an appointment with the GP or practice nurse (POH).
If you are unable to come to the practice for medical reasons, you can ask for a home visit. To request a home visit, please contact the assistant between 08:00 and 10:30. The assistant will decide, possibly together with the GP, whether a home visit will be made.
If you will be travelling soon, you can come to us for travel advice. After filling in the travel vaccination form, you can make an appointment at our practice with Friederike Bakker. She will check with you what vaccinations and medication may be needed. You will be given a prescription if necessary. For the vaccination, you can then make an appointment with the assistant.
Travel advice is only for patients registered with us.
GP care falls under the Health Insurance Act and is fully reimbursed by your health insurance, you will not receive an invoice from us for this. An exception is the traveller's vaccination, for which you will receive an invoice from us, which you must pay to us and, depending on your health insurance, can reclaim from your health insurer.
In addition, if you are prescribed medication or an additional examination is requested by your GP, this may be deducted from your own risk. In most cases, this also applies to laboratory tests (examination of blood, urine, faeces, etc.).
Vaccinations or inoculations on own request and consultations or operations in uninsured patients must be paid by the patient himself.
If you fail to attend an appointment without cancellation notice, the practice may charge for this. The charge depends on the time that was reserved for you.
Your GP and your pharmacy may share your medical data with other healthcare providers. For example, if a GP at the GP surgery or a service pharmacy needs your medical data to help you quickly, properly and safely. Sharing your medical data is done through the National Switch Point (LSP). But only if you agree.

For quality improvement, we would like to gain more insight into how our practice is performing. Your opinion is very important here, as you can judge certain aspects well.
